There is no activation fee for Apple Watch, plus your first 90 days are on us. After your free introduction, the monthly rate is $10/mo. 
Yes, each iPhone line on your plan can add up to 3 connected Apple Watches.
No, only Apple Watch GPS + Cellular has cellular connectivity capability. Only the cellular capable watch has a red ring on the Digital Crown, also known as the dial, of the watch.
Yes, Apple Watch needs to be paired to an iPhone in order to activate cellular service.
